Diabetic cardiomyopathy: Evidences, pathophysiology, and therapeutic considerations

摘要

The very existence of diabetic cardiomyopathy has always been a contentious issue. Strong evidences are now coming to fore favouring its existence. Clinical studies have confirmed the association of diabetes with left ventricular dysfunction, independent of hypertension, coronary artery disease and valvular heart disease. Though diastolic dysfunction of the LV has been regarded as a hallmark of early stage of the disease, new echocardiographic techniques have demonstrated systolic dysfunction also to be present early in the disease course. Several mechanisms have been implicated in the pathophysiology of diabetic cardiomyopathy. These include metabolic alterations, myocardial fibrosis, small vessel disease, endothelial dysfunction and cardiac autonomic neuropathy. There are no definite guidelines for treatment of the disease. However, good glycaemic control, use of ACE inhibitors and calcium blockers are viable options.
