This report describes a patient suffering from lumbar type sciatic pain from a loosened total hip replacement. Pronounced unilateral-atrophy of the iliopsoas muscle was found on magnetic resonance imaging. Under discussion as the cause of the clinical complaints is on the one hand the change in the biomechanics of the lumbopelvic hip region caused by the atrophy of the iliopsoas muscle. On the other hand, the lack of afferents from the hip joint due to damage of the joint capsule and acetabular labrum caused by the surgical technique seems to play an important role. In addition the changes in gait pattern are described, which were objectively determined by three-dimensional gait analysis.
