Background. Back pain patients often present avoidance behavior with decreasing physical activity. To counteract the avoidance behavior multiple physical training programs have been developed. Here several psychological mechanisms play a role, but their impact has not been satisfactorily clarified. The present paper addresses this relationship and examines the changes of ailment and some psychological parameters during active physical training. Methods. A total of 224 participants in a trunk muscle training program were interviewed before and after 24 training sessions. The assessment included bodily ailment and pain intensity for the ailment profile and anxiety, depression, pain behavior (avoidance, resignation, and deflection), and beliefs in control (internality, social and fatalistic externality) as psychological parameters. Results. Beyond positive changes of the ailment profile there were decreasing psychological parameters like anxiety, depression, avoidance, resignation, and social externality with effect sizes between 0.16 and 0.49. Additionally there were some significant correlations between the individual reductions. No significant changes were found for internality, deflection, and fatalistic externality. Conclusion. The present paper provides evidence for the impact of anxiety and depression reduction and a modulation of pain behavior during a physical training program in back pain patients. The study of the causal relationships should be the objective of further investigations.
