abstract_textpThe first assessment of a patient entering an emergency department ( ED) plays a central role in determining the urgency by which this patient has to be seen by a physician. Furthermore, it determines subsequent procedures and further utilization of resources. Although EDs are known to be "high-risk areas", standard protocols are rarely used for the first clinical assessment in Germany. Clinical triage systems, e.g. the Manchester Triage Scale, provide standard assessment protocols for identifying the critically ill patient who requires treatment first, especially in periods of ED crowding. The other patients are appointed to different triage categories, and thus, different waiting times. Only limited and heterogeneous data are available on reliability and validity of the different triage systems used, but good evidence emerges that argues for establishing triage systems as instruments of risk management in more German hospitals. Furthermore, from the studies it is evident that the qualified and clinically experienced nurse remains the "key player" in a well established triage system for reliable first assessment./p/abstract_text
