A 46-year-old male with neurofibromatosis type 1 presented to hospital with chest pains and dyspnea. A chest X-ray revealed a large effusion and complete shadowing of the right hemithorax. The patient was transferred to our clinic in a stable hemodynamic condition with a hemoglobin value of 4.0 mmol/l. A computed tomography (CT) scan revealed a spontaneous rupture of an aneurysm of the right 9th intercostal artery without signs of active bleeding and an extensive right-sided hematothorax. The aneurysm was treated by endovascular embolization. An anterolateral thoracotomy with removal of a massive hematoma was subsequently performed. In this report we discuss the diagnostics and therapy of vascular aneurysms associated with neurofibromatosis type 1.
