The osteosarcoma is the most frequent malignant bone tumor in children. One of its variants, the telangiectasic osteosarcoma, may be confused with aneurysmal bone cysts. The purpose of this work is to compare the clinic, morphological and imagenologic characteristics of those bone lesions, in order to improve the diagnosis. A six year old female patient who consulted for progressive swelling, pain and functional limitation in the proximal third of her left leg. The diagnosis of aneurysmal bone cyst was made initially. The radiography of proximal tibia and fibula showed expansive lesion, radiolucent to the metaphysis. The histopathological study showed central cystic and/or hemorrhagic spaces and thin septations around these spaces. Both results were interpreted as aneurysmal bone cyst. The patient evolved torpidly and it was necessary the amputation of the involved limb. A retrospective study of previous biopsies and the evaluation of the last available sample led to the final diagnosis of telangiectasic osteosarcoma. The importance of an adequate multidisciplinary correlation of musculo-skelethal tumoral lesions involving, epidemiological, clinical, radiological and histological aspects is pointed out in this study. The evolution of the patient posterior to chemotherapy was satisfactory.
