To the Editor: Vibrio metschni-kovii is a gram-negative, oxidase-neg-ative bacillus. This species was isolat-ed in 1981 from blood culture of an82-year-old diabetic woman withcholecystitis (1). It was previouslyisolated from river water, clams, oys-ters (2), fish (3), and birds that died ofa choleralike illness (4). We reportisolation of V. metschnikovii inbronchial aspirate from a patient withpneumonia.A 63-year-old man was admitted tothe intensive care unit (ICU) of A.Calmette hospital in Lille, France, foracute respiratory failure related tocommunity-acquired pneumonia. Thepatient had a history of chronicobstructive pulmonary disease with aforced expiratory volume of 820 mLin 1 s (32 of predictive value); hewas treated with oral salmeterol,terbutaline, and prednisolone (40mg/day). He was HIV negative. OnICU admission, he had the followingvalues: respiratory rate 30/min, tem-perature 39°C, pulse rate 140/min,blood pressure 140/90 mm Hg,Glasgow coma score 15, leukocytecount 13.7 ×109/L, hemoglobin level10.2 g/dL, procalcitonin level 22ng/mL, and C-reactive protein level73 mg/L. A chest radiograph showeddiffuse bilateral infiltrates. Analysisof arterial blood gases with 6L of oxy-gen/min showed respiratory acidosis:pH 7.30, pCO259 mm Hg, pO278mm Hg, HCO323 mmol/L, O2satura-tion 94. Other laboratory test resultswere normal. After noninvasive venti-lation failed, the patient was immedi-ately intubated and mechanicallyventilated
