Ergometry is one of the basic diagnostic tools in clinical cardiology. In the light of the enormous progress in cardiac imaging and risk stratification, a reassessment of the clinical utility of ergometry is reasonable. Some aspects of the recommendations for the use of ergometry have been specified. In particular, there has been a fundamental change with respect to the primary diagnostics of coronary artery disease (CAD), where ergometry is now recommended only in cases of a pretest probability of 15-65 for the presence of a stable CAD, which is in accordance with current ESC guidelines. Further modifications compared to the German Society of Cardiology (DGK) guidelines from 2000 are related to the supervision of the exercise testing by nonphysicians and the presence of a physician during the procedure, where recommendations made by the American Heart Association (AHA) in 2014 were adopted.
