Duodenal pouches even without Brunner's glands are able to dispose of large amounts of acid, presumably by a combination of H+ 'back diffusion' and bicarbonate secretion. This ability is a function of H+ concentration and hence H+ load. In order to study this mechanism, duodenal pouches 10 cm long were created distal to the duct of wirsung in 5 adult mongrel dogs. The duodenal acid disposal mechanism is dependent on acid load as determined either by H+ concentration or rate of acid delivery. This mechanism becomes less efficient as the load is increased. Some of the duodenal acid loss may be due to bicarbonate secretion since diamox inhibits carbonic anhydrase; diamox also inhibits chloride absorption which might explain less bicarbonate secretion because of chloride bicarbonate exchange.
