Colorado is requesting a waiver to allow provision of home and community - based services under Section 1915(c) of the Social Security Act for an initial period of 3 years. This waiver is requested to allow for provision of a wide array of home and community - based services (except for room and board) to eligible persons as an alternative to institutional placement in a skilled nursing facility or intermediate care facility. Colorado also requests authorization to refuse home and community - based services to eligible elderly, blind adult, or physically handicapped adult clients for whom it can reasonably be expected that the costs of home and community - based services would exceed the costs of institutional care on an amount per capita basis. Services to be provided under the waiver include case management, homemaker, personal care, adult day care, respite care, Meals on Wheels, nonmedical transportation, minor home modifications, and electronic monitoring / communication devices for persons living alone. The services will be provided as of July 1, 1982. The report discusses responsibility for services under the waiver, eligibility for services under the waiver, State assurances, client evaluation - development and implementation of plan of care, and the proposed waivered home and community - based program. Appendices provide sample forms, cost containment assurances, level of care assessment, and financial records.
