The final volume in a four-part series on the sharing of services among health care organizations analyzes the economic impact of shared services in six case studies. The report highlights economic factors to be considered by planners in assessing the potential for shared service arrangements among health care delivery organizations. Risks, detrimental situations, and undesirable consequences of shared services are pointed out and areas in which major long-range benefits can be expected from shared services are identified. Following a summary of the recommendations and principal findings from economic impact analyses performed for five shared services (personnel / collective bargaining, blood banking, laundry, obstetrics and pediatrics, and printing) in six study sites, the conceptual framework within which the analyses were performed is described. The aspects of economic impact considered in the analyses are identified, and the criteria used in selecting the aspects are discussed. Methods used in selecting case study sites and in carrying out the studies are described. The bulk of the document consists of the six case reports, each containing a summary of principal findings, historical background, analysis and supporting data, and a review of considerations for health planners. At each site, the findings show that the long-term financial commitment of major hospitals, the structure of charges for service, the perceptions of demand for expansion of patient care, and the repercussions for various physician groups appear to be related to the cost and viability of the shared service.
