Using secondary data analyses of available data, this study examines domiciliary care living arrangements (as defined by the Social Security Administration) as a potentially viable publicly supported, cost contained, and appropriate service delivery mechanism for vulnerable persons requiring long-term care services. Clients can pay for these residential care arrangements with Federal Supplemental Security Income (SSI) and State optional supplementation to the Federal payment. The care does not involve direct provision of medical and nursing care by the facility, and the facility does not qualify as a Medicaid-certified facility. The report descriptively analyzes domiciliary care by client group and funding features as well as domiciliary care's goals. It examines the extent to which program and regulatory elements impinge on the achievement of these goals and the supply and usage of domiciliary care within each State. These goals include ensuring safe and sanitary environments, having clients and their families participate in decisionmaking, using standardized procedures for assessing eligibility and implementing appropriate placement, and ensuring that SSI clients have financial accessibility to residency in a domiciliary program facility based on the SSI dollar amount received. Data for these secondary analyses cover all 50 States and the District of Columbia and were gathered through a national mail survey. The report contains numerous tables and some footnotes.
