Urban/Rural Differences in Elderly Use of Long-Term Care

The report presents the results of longitudinal and cross-sectional analyses ofdifferences between urban and rural elderly in the use of nursing home services, home health services, other community-based support services and physician and hospital services under three alternative definitions of urban and rural areas. Multivariate logistic and Tobit utilization models are estimated with data on 5,000 elderly aged 70 years and over from the 1984 and 1986 panels of the National Center for Health Statistics' Longitudinal Study of Aging and Bureau of Health Professions' health resource data. Utilization of hospital, nursing home and physician services, defined as use/no use, was found to be unrelated to urban/rural residency or the relative availability of health resources. Nor is increased utilization due to declining health status related to residential location. However, compared to the elderly living in metropolitan or totally rural areas, elderly living in the less urbanized areas are less likely to use home health care while elderly living in totally rural areas are much more likely to use community-based support services, e.g. meals-on-wheels. Finally, while urban and rural elderly are similar with respect to frequency of physicians visits, urban elderly use hospital services more frequently.
