The author studied three methods to teach breast self-examination (BSE) in a practice setting. After a pre-test, one third of 260 women patients received nurse Mammacare instruction, stressing tactile skills (MAM); one third received traditional nurse instruction emphasizing BSE technique (TRAD); and one third received no nurse instruction (CON). Half of each group received physician encouragement. Self-reported BSE frequency rose in all groups. Physician encouragement did not improve sensitivity or specificity, but women who performed BSE infrequently at pre-test and who were given only physician encouragement reported greater frequency than similiar women who received neither physician encouragement nor nurse instruction. No group increased health care use during the year after instruction. MAM instruction resulted in more long-term improved lump detection and BSE technique use than TRAD instruction or MD encouragement. BSE instruction should emphasize the development of lump detection skills.
