摘要

The study analyzes differences in physician resources use for hospitalized patients in Health Maintenance Organizations (HMOs) (network and IPAs) and commercial insurers. The research compares several measures of inpatient resource use per admission for hospitalized patients in Minneapolis-St. Paul. 1982-1984 medical record data were the main data source. Controlling for a variety of patient and household characteristics, log-linear multiple regression was used to analyze data for 10 specific conditions. The major findings were: (a) Network HMOs had shorter length of stays for six of the 10 diagnoses studies, IPAs had shorter stays for two out of 10 and lower charges for one out of 10 lower ancillary charges for three out of the 10 conditions; (b) Network-HMOs physicians treat their hospitalized patients differently and have a unique practice style in terms of resource use; (c) The competitive Twin Cities health market did not influence IPAs to reduce inpatient resource use relative to commercial insurers.
