This study responded to AHRQ’s research priority to improve care at the end of life, by testing the feasibility of an evidence-based educational intervention designed to address barriers to effective pain management in hospice. It used a group-randomized design in four hospice agencies (two intervention and two control), wherein community dwelling patient-family pairs were recruited and hospice staff in the intervention group used a brief screening instrument about barriers to pain management, provided written materials, and delivered an individualized verbal intervention to patient-family pairs upon admission, to reduce barriers to pain management. The intervention addressed common myths and misconceptions about pain management and was tailored to patients and families to address the pain-related topics that were relevant to them. Control subjects received standard hospice services. By providing hospice patients and families with accurate and relevant information about pain and pain treatments, the intervention was hypothesized to improve the management of patient pain. Hence, the aims of this project were to conduct the intervention and: (1) assess the feasibility of data collection; (2) evaluate intervention fidelity; (3) provide initial data on efficacy and derive sample size estimates; and (4) determine staff time and training costs.
