The current list of routine and non-routine laboratory tests for end-stage renal disease patients undergoing dialysis under Medicare coverage was prepared in 1978. A review of the coverage policy for these services finds that all the currently listed tests continue to be regarded as being normally required for the management of these patients. However, both aluminum and iron toxicities have recently been identified as increasing clinical problems in the group of patients. It has been suggested that the early detection of aluminum or iron overload can be achieved by routine monitoring of the serum in dialysis patients. This could lead to treatment to prevent or reverse the clinical manifestations of these toxicities which could otherwise result in serious morbidity and in some cases mortality. A review of available literature and information submitted to the Office of Health Technology Assessment suggested that frequent measurement of serum aluminum and iron be incorporated into the list of routine laboratory tests for patients on chronic dialysis.
