The Office of Health Technology Assessment (OHTA) emphasizes three basic principles in its assessment activities: (1) broad and open participation both within and outside of the Federal Government, (2) reliance on the expertise and research abilities of outside organizations and individuals, and (3) broad dissemination of assessment reports. This assessment focuses on the lactulose breath hydrogen test (BHT) for small bowel bacterial overgrowth and small bowel transit time. The use of lactulose as the test meal substrate in the BHT to measure small bowel bacterial overgrowth and small bowel transit time is appealing due to the convenience and safety of the test. However, evidence in the peer-reviewed published literature in support of the clinical utility of the test for small bowel bacterial overgrowth and small bowel transit time is lacking. No controlled, prospective clinical trails have been reported. Existing clinical studies lack the number of subjects required to account for variability within the population. Moreover, better validation is needed to explain the false negatives and positives observed. Evidence is also needed to show the usefulness of these tests in clinical decisionmaking. Contacts with medical groups did not reveal widespread clinical acceptance of the lactulose BHT for these diagnostic applications at this time.
