Intrapartum Intensive Care and Outcome of the Infant

The purpose of this study was to compare the outcome of 297 high-risk infants born in community hospitals and then transported to an Intensive Care Nursery with a similar population of 393 infants born in a general hospital where obstetric and pediatric care form a continuum throughout the intrapartum and neonatal periods. Mortality, neonatal morbidity and long-term growth and development of the two populations were also evaluated. The study concluded that the high-risk mothers and infants who were transported to the teritary obstetric service for delivery fare better than those delivered in community hospitals with Level I facilities when the infant is transferred after birth. The fetal death rate is low partly because the mother was identified as high-risk and is sent to a center where coordinated efforts in management and experience exist. Neonatal mortality and long-term outcome are also improved under these circumstances.
