摘要

On April 1, 1998, a 34-year-old male Safety Officer collapsed and lost consciousness at a training exercise. Despite cardiopulmonary resuscitation (CPR) and advanced life support (ALS) administered by the fire fighters, emergency medical technicians (EMTs), local hospital emergency department personnel, and tertiary care medical center personnel, the victim died on April 2, 1998. NIOSH was notified of this fatality on April 6, 1998, by the United States Fire Administration. On October 14, 1998, NIOSH telephoned the affected Fire Department to initiate the investigation. On October 19, 1998, a Senior Medical Officer and a Safety and Occupational Health Specialist from the NIOSH Fire Fighter Fatality Investigation Team traveled to Indiana to conduct an on-site investigation of the incident.
