Epidemiologic time-series studies conducted in cities have identified anassociation between concentration of ambient particular matter (PM) and deaths (mortality) as well as hospitalization (morbidity) among the elderly. Although these studies have raised concerns about public health effects of particulate air pollution and contributed to US regulatory decisions, questions persist about interpretation of these studies. Detractors note little consistency in data collection and analysis among the studies. The National Morbidity, Mortality, and Air Pollution Study (NMMAPS) was designed to characterize the effects of airborne particles less than 10 um in aerodynamic diameter (PM 10), alone and in combination with gaseous air pollutants. Study cities were chosen by population size and availability of PM10 data. In 90 cities, we conducted a time-series study of mortality using the same analytic approach to examine PM10. In 14 cities the investigators examined the association of PM10 with hospitalization among those 65 years of age or older. Mortality results were generally consistent with an average 0.5% increase in overall mortality for every 10 ug/m3 increase in PM10. This effect was slightly greater for deaths due to heart and lung disease than for total deaths. The largest regional effect of PM10 on mortality was seen in the Northeast. Results of the morbidity analysis were consistent with a 1% increase in cardiovascular disease admissions and about a 2% increase in admissions for pneumonia and chronic obstructive pulmonary disease for each 10ug/m3 increase in PM10.
