On March 25, 1998, about 4:48 a.m. eastern standard time, southbound Norfolk211u001eSouthern Corporation (Norfolk Southern) train 255L5, which was en route to Fort 211u001eWayne, Indiana, struck eastbound Consolidated Rail Corporation (Conrail) train TV 211u001e220, which was en route to Columbus, Ohio. The Norfolk Southern conductor was 211u001ekilled, the engineer and student engineer sustained minor injuries. The two 211u001eConrail members were not injured. The safety issues discussed in this report are 211u001ethe adequacy of Norfolk Southern's methods for ensuring compliance with the 211u001eoperating rules, the adequacy of Norfolk Southern's locomotive engineer training 211u001eprogram, and the importance of train crew resource management to safe and 211u001eefficient train operations.
