Back pain is a common condition of great social and medical cost, affecting 11211u001eof the population each year. The authors had previously conducted a series of 211u001estudies examining care patterns and outcomes for acute low back pain. In these 211u001eadditional studies, the authors examined the use of referral and multiple 211u001eprovider use among low back pain patients. The analysis was based on a 22 month, 211u001eprospective cohort study of patients with acute low back pain. Patients were 211u001efollowed after an initial visit to one of four provider types: private primary 211u001ecare physicians, chiropractor, orthopedic surgeion, and HMO primary care. 211u001eInterviews were conducted at baseline, 2, 4, 8, 12, 24 weeks, and 22 months, 211u001eSeventy nine percent of patients saw only the initial provider who began their 211u001ecare for low back pain. Logistic regression reveled that duration of pain prior 211u001eto initial visit, sciatica, higher Roland disability score, longer duration of 211u001epain after initiation of care, and referral by the initial provider were all 211u001eassociated with care seeking from another provider type. Patients inititating 211u001ecare with orthopedic surgeons were most likely to seek care from another provider 211u001etype, patients in a group model HMO the least. Among chronic LBP patients (> 3 211u001emonths), 41% sought care from an orthopedic or neurologic surgeon. Use of 211u001emultiple provider types is associated with several factors, one of which is 211u001einitial provider type. The cost of such use is significant.
