Case Managers and other long-term care professionals find themselves increasinglyhelping clients plan and prepare for their long term care needs, the topic of advance directives and an understanding of what this topic involves becomes more relevant. This brief describes 49 things that are known about advanced directives. Meant as an educational reference tool, this brief should serve as a useful starting point in educating long term care professionals and others about advance directives. An Advance directive is a health directive made while cognitively intact in order to guide a person's health care, if he or she becomes temporarily or permanently unable to make a decision. Two types of advance directives exist: a living will and a durable power of attorney. These documents are legally recognized in all states but may be legally required for a person to retain or exercise any right pertaining to the use or non-use of life sustaining treatments. These four briefing papers are on the following topics: (1) Advance Directives:49 Things That Are Known And 9 Things That Are Not Known, (2) Delegation of Nursing Activities, (3) Family Care in the Year After A Stroke or a Hip Fracture, (4) State Surrogacy Statutes for Health Decision-Making:Am Emerging Area of Law.
