The main objective of the Hospital Community Care Linkages project was todirectly affect the rate of acute care facilities' discharges to long term care institutions, and provide where medically and socially possible, community alternatives. The goals were: (1) Outposting of the preadmission staff in select hospitals to counsel specially screened patients and their families regarding the patient's possible eventual return to the community; (2) restructuring of the preadmission screening (PAS) criteria and procedures so that the PAS staff may conduct all assessment screening prior to nursing home placement and identify community placement at the earliest time following hospital admission; (3) increasing the number of temporary nursing home residents who would be returned to the community; and (4) strengthening of the formal linkages between acute care facilities and PAS, and between PAS and nursing homes.
