In late 1990's a new initiative entitled the "Community Ward Team" wasudintroduced to our Board's Community Care areas. The concept of theudCommunity Ward Teams was to provide outreach medical, nursing and paramedicaludservices on a multi-disciplinary basis in the patients own home,udthereby obviating the need for in-patient admission with the obvious resultantudsocial gain.udThe aim of this service was to reduce acute hospital admissions and to facilitateudearly discharge to an appropriate level of care in the community.udFor the first year of operation of the Community Ward Teams most areasudconfined admissions to patients who fell within the original criteria forudadmission, i.e. a multi-disciplinary team input from at least two specialities foruda fixed period of time (12 weeks) to patients capable of being rehabilitated toudlive in the community with normal community supports. Over the first year,udwhile at first admission were confined to patients who had been seen by audGeriatrician, it rapidly became apparent that General Practitioners access wouldudgreatly enhance the delivery of the service. After a brief pilot period where noudmajor difficulties were encountered in allowing General Practitioners to haveudaccess to the service, this was extended to other Community Care Areas.udSubsequent to this, it was also found that other hospital Consultants wished toudrefer to the Community Ward Teams, and this was also accommodated.
