The present invention provides a customized rehabilitation exercise using the avatar instructing the rehabilitation exercise and the avatar moving according to the user's motion by generating the user's own 3D avatar and showing the same motion by simulating the user's motion. To this end, the present invention provides a user avatar output unit for generating a 4D avatar including motion information of a user and outputting the operation of the 4D avatar to a screen; A guide avatar output unit for outputting a guide avatar for guiding performance of a specific operation to the user; There is provided a motion guide device using a 4D avatar including a user motion determination unit to determine the motion of the user by comparing the motion of the user avatar and the guide avatar. Therefore, according to the present invention, it is possible to more accurately estimate the user's movement by showing the user's figure in 4D reconstruction, thereby inducing the user to perform the rehabilitation exercise in the correct posture.
