A coal fault formation simulation test apparatus, and forward and reverse fault simulation test methods, the apparatus comprising a hydraulic lifting apparatus (5), a lower angle adjustment apparatus (6), a push rod apparatus (7), an upper portion loading apparatus (8), an upper angle adjustment apparatus (9), a lateral loading apparatus (10) and a transparent side plate (13), the hydraulic lifting apparatus (5) and the lower angle adjustment apparatus (6) being arranged below a test body (14), the upper portion loading apparatus (8) and the upper angle adjustment apparatus (9) being arranged above the test body (14), the transparent side plate (13) being arranged on a rear side surface of the test body (14), the lateral loading apparatus (10) being arranged on an end surface of the test body (14). When the apparatus is used to simulate a forward fault, the inclination angles of an inclined plate (61) of the lower angle adjustment apparatus (6) and an inclined push plate (92) of the upper angle adjustment apparatus (9) are the same, and the upper portion loading apparatus (8) loads downwardly to form the forward fault. When a reverse fault is simulated, a fault inducement apparatus is arranged at a bottom portion of the test body (14), and the lateral loading apparatus (10) performs loading to form the reverse fault. The apparatus and method solve the technical problem that it is difficult to control the fault inclination angles and initiation positions of simulated faults.
