A method, apparatus and system for acquiring a vascular assessment parameter on the basis of an angiographic image. The method for acquiring a vascular assessment parameter on the basis of an angiographic image comprises: acquiring a real-time pressure Pa at an entrance of a coronary artery in an imaging state to obtain a pressure waveform in the time domain Pa-t (S100); building a three-dimensional model for a vessel segment of interest in a two-dimensional angiographic image of the coronary artery in the imaging state to obtain a three-dimensional vascular mesh model (S200); acquiring a real-time blood flow velocity v of the three-dimensional vascular mesh model to obtain a velocity waveform in the time domain v-t (S300); acquiring a pressure-drop waveform in the time domain ΔP-t from the entrance of the coronary artery to a distal end of the coronary stenosis by means of Fourier transform and Fourier inversion; acquiring a vascular assessment parameter of the coronary artery in the imaging state. The method and apparatus enable measurement of a vascular assessment parameter of a coronary artery during diastole, and improve the accuracy thereof by means of filtering interference data by using Fourier transform.
