FIELD: medicine.;SUBSTANCE: invention refers to medicine, namely to traumatology and laboratory diagnostics, and can be used for selection of tactic of revision endoprosthesis replacement of large joints. Method involves conducting a laboratory examination of the synovial fluid samples from the joints subjected to revision endoprosthesis. Additionally, the synovial fluid samples are taken from an intact large joint. Use of local anesthesia and blood ingestion during sampling of the puncture needle is excluded. Method includes determining in all samples an expression level α-defensins HNP-1, HNP-2, HNP-3. If expression ratio is detected α-defensins HNP-1, HNP-2, HNP-3 in the operated and intact joints 2:1 and higher, the presence of infection of the operated joint is evaluated, and a two-stage revision endoprosthesis replacement with the antibacterial joint spacer installation is prescribed. If expression ratio is detected α-defensins HNP-1, HNP-2, HNP-3 in the operated and intact joints less than 2:1, the absence of infection of the operated joint is determined, and one-stage revision endoprosthesis is prescribed.;EFFECT: invention provides more accurate selection of the proposed surgical management based on a comparative assessment of the level α-defensins HNP-1, HNP-2, HNP-3 in intact and operated joints.;1 cl, 6 dwg, 2 ex
