Disclosed are a therapy guiding system based on matching between a 2D fluoroscopy image and a 3D CT image and a method thereof. According to an embodiment of the present invention, the therapy guiding method based on image matching includes: a step of obtaining a two-dimensional fluoroscopy image with respect to a preset region of interest (ROI) of a patient and a three-dimensional CT image with respect to each of a plurality of breaths; a step of extracting a feature point with respect to each of the obtained three-dimensional CT images; a step of forming a four-dimensional CT image by matching between the three-dimensional CT images using the extracted feature point; a step of selecting a two-dimensional projection image most similar to the two-dimensional fluoroscopy image through matching between two-dimensional projection images with respect to the formed four-dimensional CT image and the two-dimensional fluoroscopy image; and a step of providing information with respect to a guiding target of the region of interest based on the selected two-dimensional projection image.
