PROBLEM TO BE SOLVED: To provide a periodic polarization reversal electrode, a forming method of periodic polarization reversal structure and a periodic polarization reversal element capable of forming a polarization reversal structure which has a uniform period in a ferroelectric crystal substrate.SOLUTION: The periodic polarization reversal electrode includes: a surface electrode having plural stripe-like reversal electrodes, which are disposed on +Z plane perpendicular to a polarization direction of the ferroelectric crystal substrate, and which are electrically connected to each other and periodically extend parallel to each other; and rear face electrodes which are disposed on a -Z plane of the ferroelectric crystal substrate opposite to the +Z plane, and which is divided into plural regions by a division region which extends at an angle relative to an extension direction of the reversal electrode as viewed form the +Z plane. A voltage is applied to the plural divided regions of the rear face electrode independently across the surface electrode; thereby the periodic polarization reversal structure is formed.
