A heart disease patient monitoring method based on a primary-level medical platform, comprising: the primary-level medical platform monitors, according to a user diagnosis and treatment plan acquired from a network hospital platform, electrocardiograph information about a user within a preset monitoring period (S10); the primary-level medical platform analyzes the electrocardiograph information, compares the electrocardiograph information with standard electrocardiograph information in the user diagnosis and treatment plan and makes statistics on comparison results within the preset monitoring period (S20); and the primary-level medical platform determines, according to the comparison results within the preset monitoring period, an electrocardiograph monitoring result within the monitoring period, and sends an electrocardiograph regulating plan corresponding to the electrocardiograph monitoring result to the user, thus enabling the user to execute the electrocardiograph regulating plan (S30). Also disclosed is a primary-level medical platform. Through connection of a primary-level medical platform and a network hospital platform, relevant parameter monitoring, reminding and execution intervention can be performed on a heart disease patient according to a user diagnosis and treatment plan, thereby increasing the treatment progress and the treatment effect of the heart disease patient.
