PROBLEM TO BE SOLVED: To provide a delivery slip integrated with a detailed statement, which takes no time and labor in sticking a delivery slip even if a packing list or the like cannot be enclosed in a box, which makes delivery possible while protecting/fixing individual information such as a detailed statement even if a peelable piece of slip required for delivery is separated, and which prevents the statement from falling off during delivery as well as making it easily taken out in a simply way when necessary.;SOLUTION: This invention relates to the delivery slip integrated with a detailed statement, wherein a pseudo adhesive and a strong adhesive are separately applied to the reverse side of a first sheet of the slip, and the first sheet and the second sheet are stuck together with the pseudo and the strong adhesives. The strong adhesive is applied in a U-shaped pattern (B) on the reverse side of the first sheet. A releasable layer is provided in a U-shaped pattern (E) on the surface of the second sheet opposite to the applied part of the strong adhesive.
