PROBLEM TO BE SOLVED: To notify involved persons such as family of a patient of his/her current medical condition so as to be recognized more accurately.SOLUTION: A patient management server 10 is capable of communicating with a medical device that is carried by a patient to measure biological information of the patient and has patient identification information for identifying the patient. The patient management server 10 includes: medical condition information acquisition means 11 for acquiring the patient identification information and medical condition information that includes biological information from the medical device; address storage means 15 for storing the patient identification information and an address of a terminal of persons involved that is used by persons involved of the patient in association with each other; emergency degree calculation means 13 for calculating an emergency degree of a medical condition of the patient on the basis of the biological information acquired by the medical condition information acquisition means 11; and transmission means 16 for transmitting the emergency degree calculated by the emergency degree calculation means 13 to the address stored in the address storage means 15 in association with the patient identification information.
