A cardiac catheter has an outer sheath (5) and an inner screw catheter (8) around a needle (6a). The needle is introduced to the septum via a vein and presented to the septum wall to effect a puncture. The screw catheter (8) has an inner needle wire (6b) passage (7). The needle has a collar (12) whose advance is halted by a screw catheter end-stop (11). The screw catheter may be fixed or held at its proximal end, and the helical section (9) acts as an anchorage and absorbs the reactive forces during the septum puncture procedure.
