The apex holder (10), for use in heart surgery, has a suction system to hold it against the heart tissue and a manipulation mechanism to be linked to a manipulating arm. A suction ring (1) has a replaceable suction lip (2) to match the shape of the heart, and with a number of suction openings (26) in direct contact with the heart tissue. The connecting link to the manipulating arm has a conical spring (3) tapering within a connector (4).
