SOLUTION: This medicine history preparation system 100 is provided with an input means 10 for inputting the contents of a prescription 60, a copying means 20 for preparing the copied image of medicine instruction records 70 written by a pharmacist himself of herself corresponding to the prescription, a storage means 30 for storing the contents of the prescription and the copied image of the medicine taking instruction records for each prescription, a retrieving means 40 for retrieving the contents of the prescription and the medicine taking instruction records according to prescribed items in the prescription, and an output means 50 for preparing a medicine history book 80 by visibly outputting the contents of the prescription and the copied image of the medicine taking instruction records corresponding to each prescription.
