The instrument has a guide tube (8) allowing the axial sliding of a pin (9) for locating an isometric femoral point F and the guiding of a tubular drill for drilling a femoral insertion tunnel (4). Thee proximal end of the tube (10) is embedded in a cylindrical sleeve (11) for forming an axial canal of the same internal diameter as the tube (8) and leading from one exterior side to the other in this tube guide. The distal end of this tube (8) has a hook (14) in form of a U whose sides (15,16) are spaced away from each other, pivoting in an axial symmetrical vertical plane of the tube guide (8) along a transversal axis (17) passing through the point of intersection of the right wing (16) and the base (18) of the U. The end (19) of the right wing (15) also is displaced from an original position flush with the axis XX of the tube guide, to a final open position obtained by an angular pivoting of about (90) deg. The regulation of the distance d between the point of the hook (14) and the axis xx permits it to be adapted to the morphology of the subject to fit the guide needle to the isometric femoral point F.
