Methods and compositions are disclosed herein for diagnosing opportunistic diseases, such as MAC (Mycobacterium avium complex) infection, toxoplasmosis and similar infections, in immunocompromised individuals. The disclosed methods and compositions generally employ optically detectable agents, such as fluorescence-conjugated antibodies or fragments thereof, and two- or three- color flow cytometry to characterize specific T-cell subpopulations present in body fluid (e.g., blood) and considered herein to be predictive of opportunistic disease. Specific embodiments involve assessing CD4-CD8- double negative (DN) T-cells or γδ+ T-cells free of the Vδ2 or the Vγ9 epitope. Fluctuations of such T-lymphocyte populations advantageously provide early indication of the presence of opportunistic intracellular pathogens, even in individuals such as AIDS patients who are refractory to diagnosis by conventional means.
