Hydroxyaatite has been known to be the mineral component of bone for many years. It is however only in the last 10 to 15 years that, following initial work in dentistry, it has become possible to spray HA coatings onto a metal base in orthopaedic surgery. Clinical implantation began about 10 years ago. It is now known that HA can be attached to cobalt chrome as well as to titanium alloy. It was originally suggested that HA acted as an adhesive and thus that the attachment strength of HA to the underlying metal and the shear and tensile strengths of the HA itself were crucial. It is now thought that HA acts by inducing bone to grow onto its surface and thus to fill recesses in the metal. HA will act in this way to fill gaps as much as 5mm in width and 3mm in depth. Thus recesses machined or electrically etched into metal can be filled by bone ingrowth if they are HA coated thus obviating the need for attaching a porous surface. The advantages of directly-formed recesses are that detachment of the surface, stress risers induced by the attachment process and the additional cost of two surfacing procedures are avoided.
