Improving Hospital Inpatient Discharge Planning Process Using Lean Thinking Principles

The upward trend in healthcare costs has invigorated the pursuit for a gamut of process and productivity improvement solutions in hospital systems. This has led to the adoption of concepts such as lean manufacturing, a combination of techniques that are often leveraged to facilitate the elimination of waste and the eventual enhancement of the value derived from business processes. Despite being a technique founded in the automotive industry, lean principles have recorded widespread success in both manufacturing and service industries. Therefore, this research endeavor focuses on streamlining and improving the inpatient discharge planning process using lean principles at Virtua Health, a four-hospital community system in New Jersey, and an early adopter in the use of six sigma and lean process improvement methodologies. To ensure an effective inpatient discharge process, the need for care coordination between different care providers, the adoption of a more holistic approach to deliver patient care, enhancing the utilization of nursing staff, and moving towards more effective means of communication represent different pieces of the puzzle that need to come together. As part of the inpatient discharge planning value stream, the initial focus was spent on the bed turnaround issue and the emergency room discharge process to create a culture of 'pull'. Using various lean thinking tools and the available technology, a 40% reduction in 'patient discharge to room clean time' was achieved. Moreover, a reduction of 15 minutes in the 'patient disposition to patient admit time' in the Emergency Department (ED) was reached along with over 20 percentile increase in patient satisfaction for time spent in ED prior to getting a bed. Furthermore, the average distance a bed flow coordinator travels in a year is expected to reduce by over 300 miles.
