Task analysis is an important tool of ergonomics for understanding and improving processes at work. It has rarely been applied to surgery, especially to operative dissection. It may become an important tool for improving outcomes for patients, reducing the high cost of operating room (OR) time, and shortening the learning curve for trainees. The method used in this study is video recording of oesophageal mobilisation during laparoscopic surgery for reflux disease, and transfer to Quicktime~R format. This is displayed in two-thirds of a computer screen, with a Word 97~R table in the other third. Each video is examined in five-second segments which are described and rated for efficacy and transferred to Excel~R for coding, analysis, and charting. Charting the results clarifies differences in technique between operators of differing skills. The best technique for each type of elementary action (grasping, separating etc) can be stated as a heuristic or rule of thumb. If taught successfully, such heuristics should improve outcomes for patients, shorten the learning curve for trainees, and benefit the hospital and its staff in important ways.
