Infectious Disease Database (IDDB): An Aid For Understanding and Detecting Infectious Disease

Infectious diseases pose a major threat to global public health and international security. Existing epidemiological surveillance systems designed to track infectious disease fail to integrate the rapidly growing pool of molecular biological data to the greatest advantage. Knowing this, we set about identifying and naming the types of data that are worth remembering in a molecular biology-based infectious disease surveillance system, focusing on the host, pathogen and disease and expanding outward to encompass numerous facets of infectious disease ecology. The use of a general purpose database management system, DBMS, is a natural choice for dealing with the volume of data, its changing nature, the need for effective backup and recovery, integrity, security plus other mundane but essential data management tasks, and the need to quickly use the data in new ways for detection of outbreak or bioterrorist threats. We present a data model for a database incorporating important classical concepts modeled down to a genetic level that enables integration of molecular biological data. Applications of the model, as well as decisions regarding the design, specific vocabulary and future additions are discussed. Taken together, we believe that integrating molecular biological data with infectious disease surveillance in such a database will accomplish two main goals: improve the effectiveness of surveillance and improve our understanding of the pathogenesis and transmission of infectious disease.
