Ontario Power Generation Inc. (OPG) owns and operates 258 dams which are up to 100 years old and range in height from freeboard structures to 132m high. OPG carries out surveillance and monitoring of these structures utilizing visual observations combined with instrumentation monitoring using a standard array approach. In order to carry out effective surveillance and instrumentation monitoring, a program was initiated to reassess the surveillance requirements for all concrete and earthfill dams. As a result, a two-stage strategy was developed. The initial stage was a screening process which used a rating system to prioritize the dams most in need a review of the current practices in place. It was based on factors such as dam age, stability and condition, foundation characteristics, incremental consequences of failure, and operation and maintenance practices. The second stage was an assessment of site specific features. This latter process includes the identification of potential failure modes using Failure Modes and Effects Analysis so surveillance and monitoring programs can be developed to detect early warning of deficiencies which, if unattended, could lead to failure. Outcomes of these assessments could include recommendations to change surveillance practices, decommission instruments, alter monitoring frequency, establish threshold levels or install additional instrumentation. This paper discusses the two-stage process and demonstrates its benefits by providing examples on the application of the Failure Modes and Effects Analysis to improve surveillance and monitoring.
