This paper focuses on the "failure to follow procedure" cause of process incidents and examines potential underlying reasons that result in employees failing to follow established written procedures. Real-life common incident scenarios are used to demonstrate opportunities to improve incident investigation effectiveness. Concepts of human reliability and human factors are explored as they relate to incident investigation activities. The focus of this paper is on those human factors and human error issues that can be influenced by the findings of the incident investigation.
