Cough can have a severe impact on health status but this is often overlooked and seldom recorded. Patients may not perceive their symptoms adequately and physician may not always judge appropriately. There is a range of measures available to assess cough. Symptom scores such as the 100mm cough visual analogue scale are easy to use but are not objective. Cough reflex sensitivity measurement provides an objective assessment and information about the underlying mechanisms of cough. It is useful in the assessment of the effect of therapy. A wide range of cough reflex sensitivity is seen in healthy subjects; this limits the use of this test as a tool to determine the presence of cough. Its impracticality for use in the clinic has largely confined it to the research setting. The most promising objective cough assessment tools are the recently developed 24-hour ambulatory cough monitors. Several cough monitors are likely to be available in the near future when validation studies are complete. They are likely to transform clinical trials of antitussive drugs and may also have a use in the clinical setting to assess the effects of therapy. Finally, three well validated cough specific, health related, quality of life questionnaires have recently been developed. They are brief, easy to administer and can be used to identify and quantify the adverse impact of cough on health.
