BACKGROUND: An increasing trend in TIPS (Traverse In-Core Probe System) related equipment issues has repetitively delayed and challenged TIP inter-calibration, TIP data collection, and processing. The TIPS is used for raw TIP flux values data collection from LPRMs (Local Power Range Monitors). The data is subsequently transferred to the 3D Monicore System and LPRM GAFs (Gain Adjustment Factors) are calculated. METHODS: The CCA (Common Cause Analysis) tool was used to demonstrate that a valid adverse trend in TIPS issues existed. Additionally, the CCA determined that the adverse trend in TIPS issues was caused by obsolescence and aging of components and / or subcomponents within the TIPS DCUs (Drive Control Units). In order to address management of this cause, both an interim bridging strategy and a project effort to reverse engineer the TIPS DCUs was subsequently pursued. The bridging strategy established was to mitigate the top vulnerability (identified as the most common failure mode under the CCA). The most common failure mode was "Missing Digits in Core Limit Display." The goal of the bridging strategy was to successfully "bridge" the time period up to successful implementation of the newly reverse engineered TIPS DCUs.
