Computerized healthcare with clinical decision support systems (CDSSs) is a promising powerful approach to the aggregation and use of patient data to identify patients who would most benefit from interventions by clinicians. When CDSSs are applied effectively, they also increases quality of care, enhances health outcomes, helps to avoid errors, improves efficiency, reduces costs, and boosts provider and patient satisfaction. However, there are limited published reports describing the impact of CDSS used in routine clinical practice. Since the potential of medical decision making was first realized, hundreds of articles introducing decision support systems (DSS) have been published in the last three decades. But even today, only few systems are in clinical use, and their full potential for optimizing the healthcare system is far from realized. Prior to CDSS implementation, clinicians struggled with obtaining and documenting the data needed to support clinical initiatives. Clinician's acceptance and utilization of CDSS depends on its workflow-oriented context sensitive accessibility and availability at the point of care, and on the integration into a hospital information system (HIS). This paper outlines technical and medical aspects of a seamless integration of CDSSs for treatments in melanoma patients, into the HIS at the General Hospital of Vienna. The knowledge representation and reasoning component of the implemented CDSSs is realized with Arden Syntax and Medical Logic Modules (MLM). Experiences gained by the clinical use of the systems are used to analyze the use of CDSSs in today's routine clinical practice.
