Currently, two principles of the kidney preservation (hypothermic machine perfusion; HMP and cold storage; CS) are generally used in kidney transplantation. Both HMP and CS are developed base on the suitable conditions that can maintain the kidney function. In this study, the vascular impedance of kidney was investigated. This experiment aims to comparison the impedance between HMP and CS methods by using a kidney of pig from slaughterhouse. The protocol of impedance estimation, after the kidney was remove from pig (mixed breed pigs), it was gravity flushing to wash blood out. After that, each kidneys were measured impedance before the preservation method. For HMP method, other 3 kidneys were preserved with continuous flow of Euro's Collins preservation solution that maintain pressure around 40 ± 5 mmHg at temperature 4-6 °C for 24 hours. For CS method, 3 kidneys were kept in chamber that filled with Euro's Collins preservation solution at temperature 4-6 °C for 24 hours. After 24 hours, impedance estimation was performed. Results: from the first impedance estimation, both HMP and CS impedances were 0.957±0.95 and 1.024±0.05 mmHg min/ml, respectively. After 24 hours, the impedance of HMP method were higher than CS method (1.059±0.09 and 1.028±0.009 respectively). Conclusion, the kidney preservation using HMP show higher impedance than CS method because the autoregulation in the kidney vascular system that effect to the metabolite phenomenon. Therefore, the HMP preservation can preserving kidneys better than CS preservation.
